Understanding the Hamstring Tendons

The hamstrings are a group of three large muscles located along the back of the thigh:

  • Semitendinosus
  • Semimembranosus
  • Biceps femoris

These muscles originate at the pelvis and extend down the thigh, crossing behind the knee and attaching to the lower leg bones through tendons. The hamstrings play a critical role in:

  • Extending the hip
  • Bending the knee
  • Stabilizing movement and athletic performance
  • Supporting balance between the hamstrings and quadriceps muscle groups

When the hamstring muscles or tendons become overstretched or torn, pain, weakness, and loss of function can occur.

Common Symptoms of a Hamstring Injury

Symptoms vary depending on the severity of the injury and may include:

  • Sharp pain in the back of the thigh or buttock area
  • Difficulty walking or bearing weight
  • Pain while sitting
  • Swelling or bruising
  • Hamstring weakness
  • Muscle cramping or spasms
  • Difficulty straightening the leg
  • Audible “pop” at the time of injury
  • A balled-up appearance of the muscle in severe tears

Types of Hamstring Injuries

Hamstring Strain

The most common type of injury, caused by overstretching the muscle fibers or tendon without tearing.

Partial or Full Hamstring Tear

Occurs when part or all of the hamstring tendon or muscle fibers rupture. Full-thickness tears can lead to significant weakness and loss of function.

Hamstring Avulsion

A severe injury in which the hamstring tendon completely pulls away from the pelvis bone (ischial tuberosity).

Avulsion Fracture

In rare cases, the tendon pulls a fragment of bone away from the pelvis during injury. This is commonly associated with explosive athletic movements.
